1. Aggregate
Cohort R-7 (continental US, mixed urban/rural distribution, n = 1,488 active operators) completed 1,471 of 1,488 scheduled maintenance windows in cycle 26-04. Completion rate 98.86%, within the cohort SLA of ≥98.5%.
Seventeen (17) windows did not complete on schedule. Disposition:

2. Recovered and deferred windows
The thirteen recovered windows were rescheduled per standard procedure. Translocation, Procedure, and Redaction steps completed nominally. Post-Return surveys show no elevated recall indicators (REM-disturbance reports, persistent imagery, somatic markers) above cohort baseline.
The three deferred windows are operators currently in elevated public-visibility contexts (one electoral, two corporate-litigation). OMC-2 has reviewed and concurs that deferral risk is lower than translocation-detection risk during the deferral interval. New windows scheduled in 26-05.
3. Missed window — Subject ▆▆▆▆-7144
Subject ▆▆▆▆-7144 (R-7, Sector NE, female, cohort age 41, maintenance interval 94 days, prior window count 19, prior redaction integrity score 0.97 mean) missed her 14 Apr window.
Cause: the staging vehicle, en route to the Selection point, experienced a non-recoverable transmission fault at 02:41 local. The Selection team aborted at 02:58 per protocol (window-close minus 22 minutes; insufficient margin for Translocation and Procedure even under compressed timing).
Per the cohort SLA, a missed window for an operator with a maintenance interval ≤120 days and a prior window count ≥15 is not automatically recoverable in the next cycle. Subject 7144 is now 14 days into an overdue interval and will reach the 30-day soft threshold on 14 May 2026.
OMC-3 (Redaction QA) is requested to pre-stage a re-redaction package against subject 7144’s file, against the (non-zero) possibility of partial recall onset during the overdue interval. No analyst action required at this time. NEW-3 will receive an out-of-cycle notice only if a recall indicator is logged.
